CCP hospitalists at Albany Medical Center are providers specializing in inpatient care who will lead the medical team and coordinate care while you are in the hospital. Our comprehensive team includes physicians, advanced practitioners, care managers, and patient navigators who work closely with your primary care practice to ensure great care in the hospital and a smooth transition following your hospital discharge.

- Your CCP hospitalist will communicate with your CCP primary care provider in a timely manner, so all providers involved in your care have the information they need.
Who is this program for?
This program is currently for CCP adult patients of our primary care offices who are admitted to Albany Medical Center.
Care in the ER
If you visit the Albany Medical Center Emergency Room, you will be cared for by the Albany Medical Center team. A member of the CCP Hospital Care team may visit you in the ER. You care team will determine if you need to be admitted or if you can return home and follow-up with your CCP primary care provider.
If You are Admitted for a Hospital Stay
If you need to be admitted, your CCP Hospitalist will direct and monitor the care you receive while in the hospital.
This includes:
- Coordination of care with your PCP and Specialists
- Ordering imaging, lab work, diagnostic tests, treatments or other medical services, interpreting test results, and prescribing medications
- Working with CCP Care Managers to ensure a safe and comprehensive transition of care
Your Discharge
Discharge planning starts at admission. Our team will communicate regularly with your primary care office to ensure a seamless transition.
Key components of our process include:
- Scheduling your post-hospital appointment prior to discharge with your primary care provider or our specialized Transition of Care Program in Albany, if needed.
- Connecting you with any necessary resources such as home care services, medical equipment, and transportation to your follow-up appointment.
- Reviewing your medications to ensure you understand what’s been prescribed.
- Providing detailed patient education to ensure you understand your discharge instructions
