We know a hospital stay can be scary and confusing. Our goal is to ensure that our patients receive the best care possible in the hospital, mirroring the standard of care you receive in our offices. CCP's Hospitalists provide inpatient care for adult patients of our primary care offices who are admitted to Albany Med.

We've created a unique and transformative hospitalist program

Our program provides comprehensive, patient-centered care that you can trust during your hospital stay. Located at one of the region’s highest performing hospitals, Albany Medical Center, our hospitalist team are in the hospital 24/7 to treat adult patients of CCP primary care practices. CCP patients who visit other hospitals will continue to be cared for by that hospital’s team.

What is a hospitalist?

A hospitalist is a board certified or eligible internist or family medicine doctor who specializes in caring for hospitalized patients.

Most often, hospitalists are employed by the hospital where they work. CCP believes the best quality hospital experience is delivered by hospital-based providers (physicians, nurse practitioners and physician assistants) who are employed by Community Care Physicians and work directly in collaboration with your primary care provider.

The CCP Hospitalist team at Albany Med provides a seamless experience for you from hospital to home.

The CCP Hospitalist team at Albany Med is an expansion of our robust offerings that include Primary Care and Transitional Care Management. With this expanded service we aim to provide a seamless experience for you from hospital to home. Your CCP Hospitalist will monitor your overall health while you’re in the hospital and direct the care you receive. This includes:

  • Coordination of care with your PCP and Specialists
  • Ordering imaging, labwork, diagnostic tests, treatments or other medical services
  • Reviewing your record to avoid duplicative or unnecessary services
  • Interpreting test results
  • Prescribing medications
  • Working with CCP Care Managers to ensure a safe and comprehensive transition of care

This is a non-teaching service focused on providing exceptional patient care as part of a collaborative team.

Choose Albany Med when possible. When you arrive tell them you are a patient of Community Care Physicians.

For our patients:

If admitted to Albany Medical Center, a CCP hospitalist may be assigned to your care. If a CCP hospitalist is not available for any reason, then another hospitalist from Albany Medical Center will care for you.

Remember, if you have a medical problem that is not an emergency or life threatening, please call your primary care office first before visiting an emergency room or urgent care. Your primary care provider may be able to resolve your problem or to direct you to the proper level of care.
